We strive to provide our patients with the best patient-centered care, so your feedback is very important to us. We would greatly appreciate it if you would take a few minutes to share your experience with us online.

- Select the image to the right or www.google.com/maps . A new window will open.
- Make sure you're signed in to your Google account (if you have Gmail, this is easy!).
- Search for a Provider or their practice.
- Click Write a Review.
- In the window that appears, click the stars to provide a score. You can also write a review.
Healthgrades
- Select the image to the right or www.healthgrades.com/review window will open.
- Type in the name of your Provider.
- If they are in the Healthgrades system, a Patient Satisfaction Survey will appear.
Doctor.com
- Select the image to the right or www.doctor.com . A new window will open.
- Select search By Name option
- Locate your doctor
- Select Rate Provider button on the left side of the screen
RateMD's
- Select the image to the right or www.ratemds.com . A new window will open.
- Type in the name and location of your Provider.
- Results should appear. Scroll down to where you can complete a review.
